Make a Referral

How to make a referral

If you have a patient who you believe could benefit from support services, you can refer them directly to us in one of the following ways: 

  • email your referral form to referrals@rdns.org.nz 
  • fax your referral form to 09 589 1081
  • call 0800 736 769

Following your referral, one of our qualified staff will assess their home support requirements.

Any information you give us will be collected and used in line with our information privacy policy.
